What are you doing to LIVE WELL?
New this year, you can qualify to earn healthcare savings because of the healthy choices you're already making at home or in your community. Whether you've invested in a home gym or volunteer to walk your neighbor's dog, tell us what you do to stay healthy and we'll tell you how much you can save!

Your choice option is SELF-REPORT.
Please log onto your HealthPartners well-being account and fill out the form under “your choice” to receive credit for your completed activity. Don’t forget to check the “completed box.”

Examples can include, but are not limited to:

  • Sports League participation
  • Weight Watchers
  • Home workouts
  • Bike to work/Walk to work
  • Community Event/Community Ed Classes
  • Employee Assistance Program
  • Emotional Health Support
  • Five a day
  • Walking Club
  • Volunteering
  • Races (walking, biking, running, skiing, other)
